I know the above scene isn't technically an underwear scene, but I've just stumbled across it today and had to include it. I've not heard of the movie before, but this scene features Max Beesley masturbating with his trousers round his ankles and underwear pulled down. In addition to underwear and nudity in movies, I absolutely love masturbation scenes, even though they are few and far between.
